Features a transcription of a historical death register from the First Protestant Dutch Reformed Church in the Town of Glen, NY.

Ontario County Town Vital Records

Includes Town Vital Records for Hopewell, Gorham, Seneca and the Village of Canandaigua.  These records are on file in the towns, not the county and include birth records, death records, and marriage records from the late 1800s to the early 1900s.

New York State Department of Health Vital Records

The NY Vital Records Office holds files of birth and death certificates for the state of New York from 1881 to present day, marriage certificates 1880 to present and divorce certificates 1963 to present.  These do not include New York City.  See below for more information on NY City vital records.

New York City Vital Records are on file at the New York City Department of Health Office of Vital Records.  Birth certificates for years 1910 to present and death certificates 1949 to present are on file in this office for persons who were born or died in one of the five boroughs of New York City.  This includes Manhattan, Brooklyn, Queens, the Bronx and Staten Island.

Earlier birth and death records are available at the Municipal Archives.

New York City marriage records are on file at the NY City Clerk.

New York divorce records are on file in the county where the divorce was granted.  Divorce certificates are on file at the NY state office of vital records but the actual records and decree are on file at the County Clerk's office.
